Community Health Workers at the FFC

Working Together to Break Down Barriers to Healthcare

Our FFC team includes team members that are certified Community Health Workers (CHW). As CHWs, we work towards healing in the Lake County community through physical and mental health initiatives. By breaking down barriers to healthcare, we help our community access the care they need.

What is a Community Health Worker?

Community Health Workers are frontline public health workers that have a deep understanding of and connection with their community. The relationship that CHWs have with their community is built on trust and compassion, ensuring every individual feels valued and heard every step of the way.


CHWs create awareness, educating the community on important health issues 

CHWs empower individuals and communities to care for their health by sharing important health information and teaching them ways to care for themselves. CHWs often organize events and seminars on health issues such as nutrition, heart disease, and diabetes.


CHWs work alongside healthcare professionals to care for your health needs

Although CHWs cannot provide formal healthcare (such as diagnosing illnesses or prescribing treatments), they play a vital role in bridging gaps in healthcare delivery. By connecting individuals with the health services they need, they ensure their communities have proper access to healthcare. 


CHWs provide the support, advocacy, and informal counseling you may need

CHWs promote and encourage healthy self-care, providing individuals with the support they need during their healthcare journeys. As advocates, they help ensure members of underserved communities can navigate the healthcare system which can feel inaccessible to some.
